As a Case Manager at Centura Health, you'll advocate for patients while collaborating closely with interdisciplinary health care partners, facilities and families to ensure the best possible care experience from admission through discharge and beyond by connecting patients to resources that help them maintain and build on positive outcomes.
Utilizes clinical expertise, discretion, and independent judgment in assessing/reassessing, facilitating care coordination,?utilization management, and patient advocacy. Responsible for assuring medical appropriateness criteria are met for status?and level of care.
What You Bring/Job Requirements In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:
Associates Degree in Nursing required; Graduate of an accredited school of Nursing required;?BSN preferred
RN License required
1 year of nursing or case management experience required.
Experience working with EMR preferred.
Working knowledge of regulatory requirements and accreditation standards strongly preferred.
Physical Requirements -?Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
Hours are typically days (8a-4:30pm) but can be some evening as well- til 7p
Expectation 1 winter, 1 summer holiday
Weekend shift- 2 per month
Weekday- Need RN to be available 4 shifts per 4 weeks
